Website to report adverse reactions from a Covid-19 vaccine
UK Government/MHRA Covid-19 adverse effect (“Yellow Card”) online reporting system for public use. If you or a member of your family have suffered an adverse reaction from a Covid-19 vaccine we encourage you to log a report on this website, set up by the Government to monitor adverse events from this experimental vaccine.
UKMFA Collated Data
We have compiled a spreadsheet with information about all of the adverse events related to the Covid-19 vaccine that we are aware of to date.
We will update this regularly and would really like to hear from you if you have anything to add to this document.

COVID-19 Vaccine – Informed Consent Form

The UKMFA have produced a referenced consent form for use by doctors and their patients to aid the process of obtaining full informed consent before having a COVID-19 vaccine.
It outlines the relevant legal obligations and protections that must be addressed and also relevant medical information about COVID-19 vaccines that must be disclosed to, and discussed with, patients.
We encourage people to share this document with their GPs and also with employers and others who may be requesting or demanding vaccination.
In light of the Yeadon/Wodarg petition mentioning the possibility of cross-reactive antibodies against a placental protein being induced by a Covid vaccine we have updated the consent form to include this new information.
